WHY REHAB APPLICATION DIARY Many applications are rejected not because of the illness itself, but because of incomplete documentation. This app helps you record your medical journey over weeks and months — clearly and logically, as a record of your own that you can attach to your application. WHAT YOU GATHER • Symptoms and complaints on a scale from zero to ten (in words) • Sick leave certificates — consecutive periods of medical leave are automatically merged into a single continuous period (sick leave bundling) • Doctor visits with doctor name and clinic • ICD-10 codes with automatic formatting • optional photos of findings SICK LEAVE BUNDLING Individual sick leave certificates that follow each other chronologically are automatically merged into one continuous period. This shows at a glance how long you were unable to work without interruption. TRAJECTORY OF YOUR CONDITION You see the trajectory of your symptoms on a scale from zero to ten. Fluctuations, deteriorations and relapses are visible over time.
