The app regularly tracks progress and vitals, sending real-time updates to the patient’s doctors and care team for seamless, coordinated care. Patients can log key health metrics, such as water intake and sleep duration, while their physical activity levels are automatically tracked. This data is shared with doctors in real time, enabling more personalized and responsive care. Additionally, patients can complete surveys sent by their physicians, providing deeper insights to support ongoing health management. To access MoveSmarter Health, download the app for free. You will then need an invitation code, which includes a secure, personalized digital code sent to your email by your physician. The email will also contain the institutional code provided by your affiliated clinic, ensuring seamless registration and access.
