Our purpose is to give individuals control of their healthcare information, allowing them to partner with providers to live healthier lives. MiPHR has developed a mobile application to address the rising cost of chronic condition management including diabetes, hypertension, and obesity. The mobile application is designed to manage patients diagnosed with chronic conditions, connecting them with their participating providers to received recurring reports for remote physiological management and chronic care management. Subscription Plans (Yearly, Auto-Renewable): Premium Plan: Unlock all features, including sending reports to your selected healthcare provider. ($12.99 per year) Standard Plan: Access all features except report sharing with participating healthcare providers. ($2.99 per year) Payment and Renewal Information: Payment will be charged to your Apple ID account at the confirmation of purchase. Subscription automatically renews unless it is canceled at least 24 hours before the end of the current period. Your account will be charged for renewal within 24 hours prior to the end of the current period. You can manage or turn off auto-renew in your Account settings at any time after purchase.
