The mobile and desktop app offers several important features to providers such as: Collates clinical and other relevant data from various sources (EHRs, labs, imaging systems, health plans, etc. as well as data entered directly by patients through free-text chat messages); Generates an in-depth view of the patient’s complete health status including history, current clinical status, social determinants, adherence, etc. using advanced CDS, NLP and machine learning technologies; Continuously analyzes new information against standards and guidelines Identifies early signs of clinically significant deviation from the patient’s steady-state; Provides real-time notifications to help clinicians reduce the risk of deterioration; guides action when needed, while limiting alert fatigue; Connects providers with patients, the patient’s other providers and authorized members of the care team for seamless, coordinated care; Generates an Intelligent Medical Summary™ (IMS) with the most relevant and current clinical information, updated in real-time for efficient access to the patient’s key clinical data.
