"Conscious" is a closed data collection and self-monitoring tool for adult participants in research programs of partner centers. The application is not intended for public use. Access is provided only to invited participants after verification of the personal code received from the authorized center. PARTICIPANT'S OPTIONS record their own observations of well-being, mood and sleep; fill out questionnaires and questionnaires of the research program; keep a private diary; receive reminders about planned research activities; download materials provided by the program; with separate consent, transfer available indicators from compatible wearable devices; view the history of their own records and responses; correspond with the center coordinator on organizational issues. RESEARCH PURPOSE The data helps the team evaluate the product's performance and explore aggregate patterns. If provided for in the participant information and with separate consent, depersonalized and aggregated results may be used for the preparation of scientific materials. Identified private participant records are not published. NOT A MEDICAL APPLICATION “Svídmi” is not a medical device and is not intended to diagnose, treat, cure or prevent any disease or condition. The application does not provide medical, psychological or therapeutic advice, does not establish a diagnosis, does not recommend treatment, medications or clinical interventions and does not make decisions in place of a specialist. Questionnaires and automatic summaries are used only for the structured collection and display of information within the framework of the research program. If the AI module is activated in the application, it only structures or briefly summarizes the information provided by the participant. It does not form medical conclusions, risk assessments, treatment recommendations or personalized clinical advice. PARTICIPATION AND CONFIDENTIALITY Before receiving the code, the partner center provides information about the purpose, duration, procedures, data processing and the procedure for terminating participation. Participation is voluntary. The participant can stop using and submit a request for their data through the center or support service. The application does not contain advertising, subscriptions or purchases, does not use data for advertising tracking and does not sell personal data.
