DentalPlus Remote Forms v4 is a tablet-optimised signing and form completion app for dental practices. It replaces paper-based workflows with a fast, digital experience — allowing patients to update their details, complete medical history questionnaires, sign treatment plans, capture photos, and provide consent, all on a tablet at the point of care. Designed to work with the DentalPlus practice management system, the app requires no patient login. Staff initiate a session by scanning a QR code or using the automatic device polling mode, and patients simply complete their forms and hand the tablet back. KEY FEATURES Patient Details Patients can review and update their contact information, address, and emergency contacts, pre-populated from their existing record. Photo Capture Take patient photographs using the front or rear camera with preview, retake, and automatic compression before upload. Medical History Digital medical history questionnaire with yes/no questions, follow-up detail fields, and signature capture at the end. Treatment Plans Patients view itemised treatment costs, agree to the plan, and provide a signature — all in one screen. NHS FP17PR Declaration Complete the NHS FP17 patient declaration including ethnic group selection, exemption eligibility, and patient signature, fully compliant with NHS specifications. Custom Forms Practices can create their own forms with 12 field types including text, numbers, dates, checkboxes, dropdowns, ratings, and signature fields. Document Signing Review and sign documents, consent forms, and staff-completed assessments. Contact Preferences Patients set their communication preferences for reminders, recalls, and marketing via SMS and email. Signature Capture Full-width, touch-optimised signature pad integrated into every form that requires it. Works with both finger and stylus input. QR Code Scanning Staff generate a QR code in DentalPlus and patients scan it on the tablet to begin their session instantly. Auto-Poll Device Mode Configure the tablet as a named device (e.g. "Reception iPad") that automatically receives signing requests without QR scanning. Ideal for dedicated reception tablets. Form Queue & Progress Tracking Patients see all forms they need to complete with clear progress indicators and automatic advancement to the next form. DESIGNED FOR TABLETS Large touch targets, readable fonts, and a portrait-only layout make the app comfortable to use at arm's length. SECURITY All data is transmitted securely via HTTPS. Sessions use one-time access tokens that expire automatically. No patient credentials are stored on the device.
